Any item you purchase can be personalized with your name.
The charge is $12.00 per item.
All names will be embroidered RIGH CHEST with BLOCK lettering and will match the embroidery of your item.
At the end of the apparel list, you will find a Personalized Item. Select that and follow the prompts.

HOW WE DO IT
Even though you are prompted to enter your shipping information, your order will not be shipped directly to you. Your order will be delivered to Healthmark.
